FINANCIAL POLICY
Thank
you for choosing us as your dental care provider. We realize that every
person’s financial situation is different. For this reason, we have
worked hard to provide a variety of payment options to help you receive
the dental care needed to enjoy excellent dental health.
Dental Insurance
You
are directly responsible for all charges, regardless of decisions made
by your Insurance Company. For patients whom we have verified
eligibility with an insurance provider, we are happy to file the
necessary forms with your insurance company to enable you to receive
your maximum of benefits possible. However, the insurance policy is a
contract between you, your employer and your insurance company; we make
no guarantee of any coverage or payments from your insurance
provider. We will estimate your insurance coverage before each dental
treatment but the final decisions will be made by your insurance
carrier. Any amount that is not covered by insurance will be due from
you at your dental appointment and your final statement will reflect a
credit or balance due after your insurance has made their payments.
Usual & Customary Rate
Our
practice is committed to providing the best treatment for our patients
and we charge what is usual and customary for our area. You are
responsible for payment regardless of any insurance company’s arbitrary
determination of usual and customary rates.
Missed Appointments
Unless
cancelled at least 24 hours in advance, our policy is to charge a $25
fee. Please help us to serve you better by keeping your scheduled
appointments.
Payment Options
Cash or Check
We are happy to offer a 10% pre-pay courtesy for all treatment paid in full, with cash, check or credit card at the time of treatment.
Credit Cards
We will gladly accept payment by MasterCard, Visa, Discover and American Express.
Monthly Payments
By arrangement through Care Credit installment loans are available. Approval is usually available within 24 hours.
Finance Charges
A finance charge of 1% will be added to all accounts 60 days overdue.
